We actively start discharge planning upon admission so that the transfer from Riverside Manor to home is a celebratory, positive and rewarding experience for patients, family members and caregivers. Throughout your stay, our case manager is in continuous communication with insurance companies and payers to convey updates and progress. Upon discharge our case manager arranges community resources of your choice and orders necessary equipment to support home success. Riverside Manor provides individualized patient and family education to promote knowledge, skills and increased home readiness.

cooked meals with locally inspired menus and fresh foods, daily housekeeping services, laundry and linen services, business office support with insurance expertise and much more.
